- Contact information: contact person (required), organization, email address (required) and phone number.

- These two statements:
- “I have a good faith belief that the disputed use is not authorized by the copyright owner, its agent, or the law.”
- “I affirm, under penalty of perjury, that the above information in this notice is accurate and that I am the copyright owner or authorized to act on the copyright owner’s behalf.”
- Your signature (typing your full name in an email counts).
What happens next
We will disable access to the challenged material as soon as possible. We will then notify you and the entity that posted the material as soon as possible to discuss resolution of the issue. If you have any questions or need additional assistance, please contact us.
